Patient Eligibility
Patients are eligible if they:
- Are uninsured or underinsured, and
- Have a household income at or below 300% of the federal poverty level.
Patients complete their own eligibility process directly with the pharmacy.
Sending Prescriptions to the Pharmacy
1. check the available medications
Before prescribing, please review the medication list to confirm availability.
If you do not see a medication listed, you may contact the pharmacy to discuss alternatives.
2. write the prescription or send electronically
You may send the prescriptions to us in two ways:
Option A: Electronic prescribing (preferred)
When e-prescribing, search for:

Option B: Give the patient a paper prescription
If e-prescribing is not available, patients may bring a signed prescription to the pharmacy. No special forms or additional document is required.
How to Tell Your Patient About Us
You may share the following information with patients:
- PMMCP provides free medications for those who qualify
- The pharmacy is located inside Gleaners Food Bank of Indiana, 3737 Waldemere Ave, Indianapolis 46241
- Patients can print out and fill out their the eligibility and HIPAA form prior to picking up their medications but do not need to
