Am I Eligible?
Most patients qualify if they are uninsured or underinsured and have a household income at or below 300% of the Federal Poverty Level.
To find out quickly, use our short online questionnaire:
If you’re not sure or need help, our team can walk you through it when you visit.
First time at the pharmacy? Here’s what to expect
1. Please bring:
- Your completed forms (optional – we also have copies available at the pharmacy)
- Your paper prescription, only if your provider did not send it to us electronically
You do not need a photo ID, income documents, or a medication list.
2. Check In
When you arrive, you’ll be asked for:
- Your name
- Your date of birth
- Your paper prescription, if needed
3. Eligibility Review
Our team will review your information and confirm that you qualify for our free pharmacy services. If you have any questions during this step, we’ll walk you through everything.
4. Pick Up Your Medications
If your prescription is available, you may receive it the same day. Some medications may be ready on a later visit. Our pharmacist will meet with you to provide counseling and help you understand how to take your medications safely.
